Low-cost sports physicals available at MHS June 10th


Western Wisconsin Health will be at Menomonie High School on June 10th from 9 am - 12 pm, providing low-cost sports physicals. NOTE: Students from any school can attend any of the scheduled clinics.

Students registering for athletics must have a current physical examination on file in the activities office before the student can participate in practice/tryouts.


Physicals are required every two years. Parents can check to see what is on file for their child(ren) through Skyward. Log in to your Skyward account and click on the Health Info button. The highlighted dates below are the dates of the last physical on file. In the example below, the second child needs a physical and the first child does not.



Per the WIAA athletic participation permit card, any physical examination taken April 1st and thereafter is valid for the following (2) school years; physical examination taken before April 1st is valid only for the remainder of that school year and the following school year.


Example: If your child had a physical April 1, 2025, or after, the physical will be good through the 2026-2027 school year.


If your child had a physical before April 1, 2025, the physical will be good through the 2025-2026 school year.


If your child had a checkup recently, contact the healthcare provider and ask about getting a completed physical form. The required form can be found HERE.
